Forms
Application for Insurance Cover (2 pages) This form is to be used by all members requesting additional insurance cover for Death Only, Death and TPD or Income Protection Insurance. Members completing this form must also complete a Short Personal Statement (Form A).	Direct Debit (2 pages) Use this for personal contributions to super through direct debit from your account.
Short Personal Statement (Form A) (2 pages) If you are requesting additional Death or Death and TPD cover, or Income Protection cover you must also complete this form.	Fee Comparison Request (2 pages) Use this form to request a comparison between the fees of Tasplan and another fund.
Insurance Transfer Form (2 pages) This form is for members that want to transfer insurance cover from other employer sponsored superannuation accounts into Tasplan.	Lost Super Search Request (1 page) Use this form to request Tasplan to search for your lost super.
Occupational Rating Form (2 pages) This form is for members that have not been occupationally rate for insurance purposes or who have changed their occupation and are now able to be rated as White Collar or Professional.	Rollover Request Use this form to rollover other funds into your Tasplan account.
Application to Reduce or Opt Out of Cover (1 pages) Members that wish to amend their level of insurance cover need to complete this form.	Superannuation Fund Nomination (1 page) Use this form to tell the Government into which superannuation fund account you want a benefit to be paid.
Application to split contributions (2 pages) Use this to transfer eligible superannuation contributions from your Tasplan account to the account of a spouse.	Tasplan Compliance Statement (1 page) An employer keeps this as evidence of Tasplan being a complying fund.
Binding Death Benefit Fact Sheet (2 pages) Facts about the Binding Death Benefit Nomination form.	Tasplan Pay Deduction Form (1 page) This form can be used to authorise an employer to make deductions to Tasplan from a member's pay.
Binding Death Benefit Nomination Form (2 pages) This form can be used to make, update or cancel a Binding Death Benefit nomination for your Tasplan benefit.	Tax File Number Notification (1 page) Use this form to give us your Tax File Number.
Change of Member Details (2 pages) Use this form to advise Tasplan of any change to your name, address or beneficiaries.	Standard Salary Sacrifice Agreement This is a template of an agreement between an employee and employer to begin salary sacrifice. Consult with your employer over the availability of salary sacrifice.
Choosing Tasplan as your fund (1 page) Use this form to advise your employer that you want superannuation contributions paid into your Tasplan account.

The Employer Guide (16 pages) contains all the information and forms you need to join Tasplan as a participating employer. The employer guide should be read in conjunction with Tasplan's Product Disclosure Statement (PDS) (48 pages, 1.1 MB). NOTE: the PDS is 48 pages and can take some time to download, depending on your system. Please contact us on Freecall 1800 005 166 to obtain a copy.		The Product Disclosure Statement (PDS) contains all the information employees need to know about becoming a member of Tasplan, including the Application for Membership form. NOTE: the PDS is 52 pages (1.9 MB) and can take some time to download, depending on your system. Please contact us on Freecall 1800 005 166 to obtain a copy.
SUPERClear (4 pages) This Brochure gives an overview of how the SUPERClear clearing house operates.		The Member Investment Choice Booklet (MIC) (12 pages) explains how investment choice works, where each option is invested, and includes a form for members to take-up or switch their investment choice. SUPERClear PDS (21 pages) This document provides all the information and forms you need to register for the SUPERCLear clearing house service. You will also need to complete the Direct Debit form attached to the PayDeduct PDS.
PayDeduct PDS (9 pages) This PDS provides all the required information and forms you need to register for the Direct Debit facility that must be set up in conjunction with the clearing house operations provided by SUPERClear.		The Tasplan Insurance Booklet (28 pages) This booklet outlines the insurance cover available to members. It details the various insurance options available and the associated costs involved.
